Privacy Policy

Last updated: September, 1st, 2026

Union Grove Family Dental ("we," "us," or "our") respects your privacy. This Privacy Policy explains how we collect, use, disclose, and protect information when you visit https://www.uniongrovefamilydental.com/ (the "Website"), submit a form, or otherwise communicate with us through the Website.

This policy applies to the Website only. It does not apply to information collected through other channels or to websites operated by others.

Information We Collect

Information You Provide

You may provide information when you submit a contact, appointment-request, or other form on the Website. Depending on the form, this information may include:

  • Your name;
  • Your telephone number;
  • Your email address; and
  • The contents of your message and any other information you choose to provide.

Please do not use a general website form to send Social Security numbers, financial account information, detailed medical records, or other highly sensitive information unless the form specifically states that it is intended and secured for that purpose. The Website is not intended for emergencies. If you are experiencing a medical emergency, call 911 or seek immediate medical assistance.

If you submit personal information about another person, including a child, you represent that you are authorized to provide that information and permit us to use it as described in this policy.

Information Collected Automatically

When you use the Website, we and our service providers may automatically collect certain technical and usage information, including:

  • Internet Protocol (IP) address and cookie or device identifiers;
  • Browser type, device type, operating system, and language settings;
  • Approximate location derived from an IP address;
  • Referring and exit pages;
  • Pages viewed, links clicked, and other interactions with the Website; and
  • Dates, times, and duration of visits.

We may collect this information through cookies, tags, scripts, and similar technologies. Although this information generally does not identify you by name, it may be considered personal information under some laws.

How We Use Information

We may use information to:

  • Respond to questions, messages, and requests;
  • Process requests for information or services;
  • Operate, maintain, and improve the Website;
  • Understand how visitors use the Website;
  • Detect, investigate, and prevent security incidents, fraud, or misuse;
  • Maintain business and legal records;
  • Comply with law, legal process, and professional obligations; and
  • Protect our rights, safety, property, patients, users, and others.

Google Tag Manager and Google Analytics

The Website may use Google Tag Manager to manage website tags and Google Analytics to understand Website traffic and usage. Google Analytics may use cookies or similar technologies to collect information such as a device or cookie identifier, approximate location, browser and device information, pages visited, referring sources, and interactions with the Website. Google may process this information on our behalf to provide analytics reports and related services.

We do not intentionally send names, email addresses, telephone numbers, message contents, or other form-field values to Google Analytics. Information processed by Google is subject to Google's own terms and privacy practices. You can learn more by reviewing Google's Privacy Policy and how Google uses information from sites or apps that use its services.

You may be able to limit cookies through your browser settings. Google also offers a Google Analytics Opt-out Browser Add-on. Blocking cookies may affect some Website features.

Spam and Abuse Protection

We use automated security tools to distinguish human visitors from automated traffic, prevent spam and abuse, and protect Website forms. Depending on the configuration of this Website, these tools may include Google reCAPTCHA or Cloudflare Turnstile.

Google reCAPTCHA

When Google reCAPTCHA is used, Google may process information such as your IP address, browser and device characteristics, interactions with the Website, and other security and risk signals. reCAPTCHA also sets a necessary cookie named _GRECAPTCHA when it runs. We use reCAPTCHA information only for security, fraud prevention, and abuse prevention.

Cloudflare Turnstile

When Cloudflare Turnstile is used, Cloudflare may process limited security signals such as your IP address, user-agent header, TLS fingerprint, browser characteristics, the Website's site key and origin, and the result of a security challenge. Turnstile is used to detect and block bots. According to Cloudflare, Turnstile does not access, store, or transmit form entries, communications, or other information you type into the Website. For more information, review Cloudflare's Turnstile Privacy Addendum.

Maps and Location Services

The Website may display maps or location information using Google Maps Platform or an interactive map built with Leaflet and map data or tiles supplied by OpenStreetMap or another provider. When a map loads, the map or tile provider may receive technical information such as your IP address, browser and device information, the requested map area or location, request URL, and date and time of the request.

If the Website offers a feature that uses your device's precise location, we will request permission before accessing it. You can deny or withdraw location permission through your browser or device settings.

Google Maps information is processed according to Google's Privacy Policy and the Google Maps/Google Earth Additional Terms. If OpenStreetMap Foundation services are used, information is processed according to the OpenStreetMap Foundation Privacy Policy. A different map or tile provider's privacy policy applies when that provider is identified on or near the map.

Call Tracking and CallRail

The Website may use CallRail to measure how telephone calls and other inquiries are generated and to help us respond to them. CallRail may use cookies, JavaScript, and dynamic number insertion to display a tracking telephone number and associate an inquiry with information about the visitor's interaction with the Website.

Depending on the CallRail features enabled, CallRail may collect or process:

  • Your telephone number and name shown through caller identification, when available;
  • The tracking number called, call date and time, duration, routing information, and general call location;
  • Referral source, landing page, pages viewed, search or advertising keywords, campaign parameters, and device type;
  • Call audio, voicemail messages, transcripts, summaries, classifications, or other call analysis when recording, transcription, or related features are enabled; and
  • Information submitted through Website forms when CallRail form tracking is enabled.

Where call tracking is used, we use this information to attribute inquiries to marketing sources, route and respond to calls, evaluate service, train personnel, maintain security, and improve our Website and operations. CallRail processes this information on our behalf as a service provider. For more information, review CallRail's Privacy Notice.

Calls placed to tracking numbers may be recorded or transcribed when those features are enabled. Where required by applicable law, we provide notice and obtain consent before or at the beginning of a recording. If you do not want a call recorded, tell the person who answers or use another available contact method.

How We Disclose Information

We may disclose information:

  • To vendors and service providers that support website hosting, maintenance, security and spam prevention, email delivery, form processing, call tracking and routing, maps and location services, analytics, and other business operations;
  • To professional advisers, such as attorneys, accountants, auditors, and insurers;
  • To government authorities or other parties when reasonably necessary to comply with law, legal process, or professional obligations, or to protect rights, safety, and property;
  • In connection with a merger, acquisition, financing, reorganization, sale of assets, or similar business transaction; and
  • At your direction or with your consent.

We do not sell or rent personal information. We do not share personal information for cross-context behavioral advertising, and we do not use information submitted through Website forms for targeted advertising.

Data Retention

We retain personal information only for as long as reasonably necessary for the purposes described in this policy, including to respond to requests, maintain appropriate business and patient records, satisfy legal and professional obligations, resolve disputes, and enforce agreements. Retention periods vary based on the type of information, why it was collected, and applicable requirements. Analytics information and CallRail communication records are retained according to our account settings, business needs, and applicable requirements.

Data Security

We use reasonable administrative, technical, and physical safeguards designed to protect personal information. No website, transmission, or storage system is completely secure, however, and we cannot guarantee absolute security.

Medical and Health Information

If Union Grove Family Dental is a health care provider or other entity subject to the Health Insurance Portability and Accountability Act (HIPAA), protected health information that we receive as part of providing health care or related services is governed by our Notice of Privacy Practices and applicable health-information laws. This Website Privacy Policy does not replace or modify that notice.

Submitting a Website form does not establish a provider-patient relationship. Unless a form specifically says otherwise, do not use a general Website form to send detailed or urgent medical information. Contact the practice directly or use its designated patient portal for patient-specific communications.

Your Privacy Choices and Rights

You may ask to access, correct, or delete personal information you submitted through the Website. Depending on where you live and which laws apply, you may also have rights to confirm whether we process your personal information, obtain a portable copy, opt out of certain processing, or appeal a decision about a request.

To make a privacy request, call 262-878-1500 or use Contact and identify the request as a "Privacy Request." We may need to verify your identity before completing a request. We will respond as required by applicable law. Certain information may be exempt from a request, including information we must retain for legal, professional, security, billing, or recordkeeping reasons.

We will not unlawfully discriminate against you for exercising an applicable privacy right.

We have not sold these categories of personal information during the preceding 12 months. We have not shared them for cross-context behavioral advertising. We do not knowingly sell or share the personal information of consumers under 16 years of age.

We do not disclose personal information to third parties for their own direct-marketing purposes.

We may request information reasonably necessary to verify the request and the agent's authority. If we cannot verify a request, we may be unable to fulfill it.

Browser Privacy Signals

Some browsers transmit "Do Not Track" signals. Because there is no universally accepted standard for these signals, the Website does not currently respond to Do Not Track signals. Third parties, including analytics providers, may collect information about your online activity over time and across different websites when you use the Website.

Where required by applicable law, we process legally recognized opt-out preference signals, such as Global Privacy Control. Because we do not sell personal information or share it for cross-context behavioral advertising, such a signal does not change our current practices.

Children's Privacy

The Website is intended for a general audience and is not directed to children under 13. We do not knowingly collect personal information online from children under 13 without consent required by law. If you believe a child has provided personal information through the Website, please contact us so we can review and, when appropriate, delete it.
